  • Monday—Thursday: 9:00 am—4:00 pm; closed for lunch 12:00 pm-1:00 pm
  • Friday: 9:00 am-11:45 am

Substance Abuse Counseling:

  • Wednesdays 8:30 am-10:30 am (counselor on Decatur campus only)

Once you have scheduled your first appointment, you can download your Intake and Informed Consent paperwork as a fillable PDFs. These can be emailed to leslie.shelor@593c18ac56.nxcli.net or printed and brought to the office in person. If using a mobile device, Adobe Fill and Sign is a safe, free app available for Android and IOS that will let you complete the form from your phone. If you are unable to complete the paperwork before your appointment, please arrive 15 minutes before your scheduled appointment time to complete in the office.
